Medical practices that use HealthCura can send their patients appointment reminders by text message. This page shows how a patient agrees to receive those texts. A patient receives them only after agreeing.
1. Where patients agree
Patients agree on the practice's registration form, which the practice sends them before their first visit. The form's "Consents, Authorizations & Assignment of Benefits" section includes a text-message clause, shown in full; the patient reads the section and initials it. The form is the practice's own, matching its paper registration packet, which patients may complete instead. Each agreement is recorded with the patient's initials, signature and date.
2. What the patient sees and agrees to
This is the text-message clause of the registration form, exactly as worded, with the initials the patient enters for the section. The practice's own name appears where "Riverside Foot & Ankle" is shown here; that name is only an example.
10. SMS Text Message Reminders: I agree to receive automated SMS text-message appointment reminders from Riverside Foot & Ankle at the cell phone number I provided above. I understand that message and data rates may apply, that consent is not a condition of treatment, and that I may reply STOP at any time to opt out.
Initial — I have read and agree to the below policies
INITIALS
3. The messages
- What they are. Reminders of upcoming appointments, a confirmation when the patient replies, and replies to messages the patient sends the practice. No marketing.
- What they contain. The patient's first name, the practice's name, and the appointment's date, time and location. Never a diagnosis, treatment details or billing information.
- How often. Message frequency varies, typically one to three messages for each appointment, plus replies to messages the patient sends.
- Cost. Message and data rates may apply.
An example reminder:
Riverside Foot & Ankle: see you Tuesday, March 3 at 9:30 AM with Dr. Lee, Main Street office. Reply C to confirm, R to reschedule. Reply STOP to opt out.
4. Stopping, starting and help
- Reply STOP to any message to stop text messages from that practice. One message confirms the opt-out, and no others are sent.
- Reply START to receive them again.
- Reply HELP for help, contact the practice directly, or email support@healthcura.ai.
Mobile carriers are not liable for delayed or undelivered messages.
5. Privacy
Mobile numbers and text-message consent are not sold or shared with third parties for marketing. See our Privacy Policy and the text-message section of our Terms of Service.
